An orange and orange juice share an obvious claim: one comes from the other. But we have changed the way we eat that food. In nutrition, form sometimes changes more than a '100% natural' label is prepared to admit.
When we eat a whole orange, we get water, natural sugars, vitamins, plant compounds, and the structural integrity of the fruit with its fibre. We have to peel it, chew it, and eat it piece by piece. When we juice it, part of the fibre and structure is left behind, and several pieces of fruit can be consumed in a minute.
The sugar was not created by squeezing. It was in the fruit before that. But its context has changed.
The World Health Organization therefore classifies sugars naturally present in fruit juices as free sugars, while treating the sugars within the structure of whole fruit differently. WHO recommends that free sugars remain below 10% of total daily energy intake, with a further reduction below 5% potentially offering additional benefits.
This does not mean a glass of 100% juice is the same thing as a fizzy drink.
Juice can still contain vitamin C, potassium, folate, and bioactive compounds from the fruit, depending on variety and processing. If the choice is between a sweetened soft drink and a small glass of 100% fruit juice, it is not reasonable to pretend they are nutritionally identical simply because both contain sugar.
But it is equally unreasonable to count a large glass of juice as fully equivalent to several pieces of whole fruit.
A more recent WHO guidance on healthy eating frames this fairly plainly: juice can be part of a diet, but most varieties — including those with no added sugar — contain a significant amount of free sugars and intake should be limited. For most fruit consumption, whole fruit is preferred.
Long-term observational studies produce a similar, if not entirely uniform, picture. Large American cohort studies have linked higher consumption of certain whole fruits with a lower risk of type 2 diabetes, while greater intake of fruit juice was associated with higher risk. A more recent 2025 meta-analysis of prospective cohorts found no statistically significant increase in risk for 100% fruit juice, but also found no protective association of the kind seen with whole fruit; drinks that were not 100% juice were associated with greater risk.
That is an important nuance.
Nutrition does not have to choose between "juice is healthy" and "juice is liquid sugar." A small glass of quality juice alongside a meal can exist quite normally within a diet. A litre of juice drunk instead of water, justified by an orange on the label, is a different matter.
Here satiety re-enters the picture. Three oranges on a plate look like food. Three oranges in a glass look like a drink. It is very easy to consume the energy of several pieces of fruit alongside a full breakfast, whereas eating the same fruit whole would feel like a substantial part of the meal.
A smoothie sits somewhere in between. Blending whole fruit retains much of the fibre that classical juicing removes. But the physical structure of the food has still been altered and rapid consumption of a larger quantity has been made easier. There is no need to declare the blender an enemy; it is simply worth not believing that the machine has no effect because the ingredients have the same names.
At VERA we do not think juice should be banned. Good juice is enjoyable and can be part of breakfast.
We just do not count it as a reason why fruit is no longer necessary.
If you want an orange, the best way to get everything an orange does is fairly old-fashioned: eat the orange.
If you want juice, drink juice.
Food works better when it does not have to pretend to be something else.
